I had a similar situation recently - but as the position in question was
voluntary we were not charged - but it did serve to highlight the ease
to which the form was processed, albeit not fully, without any
authorisation from ourselves.

Subject: Phone applications for Disclosures
Dear Collegues,
We are currently disputing the payment of part of an invoice with the
CRB. A former students phoned the CRB, quoting our reference number
(which we did not give him but which he could have got from a friend's
CRB application form), and requested a Disclosure application form,
which the CRB have begun to process. This was neither requested nor
authorised nor countersigned by us; the individual in question no longer
has anything to do with our institution; but the CRB are insisting we
pay for this Disclosure.
Is this a situation that anyone else has encountered? It strikes me as
ludicrous that someone can set the CRB application process in motion
without any contact with an institution, incurring a cost to that
institution and undermining its security procedures. I believe it should
be impossible for an application even to be set in process without a
countersignatory's approval. I'd be very grateful for any
comments/experiences you were happy to share.
