New Falcon out of Tempe, AZ puts out some interesting stuff, including
much of Christopher Hyatt and L.M. Duquette's work. If only they could
learn to make a binding that didn't disintegrate the first time you read
the book. Don Weiser, Samuel Weiser's son, still runs a
antiquarian/occult shop up in York Beach, ME and I believe he puts out
some limited edition books as well. Destiny Books is often good as well.
