Strangely enough, my colleagues and I were discussing record keeping and
history taking at our clinical supervision meeting yesterday. One of our
group shared an experience of how she was so relieved that she had kept
really good written records of a particular case. She used a validated
model of record keeping using SOAP (subjective, objective, action, plan). I
don't know enough about this yet to give you any more details, as I had it
as a personal objective for me to do further research into this. Have any
list members used this method? What are your experiences of it? Is there
any training on it?
