The answer is that DM appears at two points in the MeSH tree: Nutritional & Metabolic Diseases and Endocrine System Diseases. I was only looking at the former, while under the latter are more, different subject headings. The total number of records between the two sections is rather more than the 86000 I was looking for as some come under more than one heading.
As Amy says, 'The trick is to click on the orange contexts button on top left corner of the screen under where it says Tree for Diabetes Mellitus.' This brings up a display of all occurences of the search term in the MeSH tree - and the 'Contexts' button changes to 'full tree'.
Now the admission: if you read the 'Hints' section below the tree display, the second hint says, 'The tree displays the search term in the first context where it appears in the full subject tree. Use the Contexts button to display all instances of where your term appears in a subject tree.'
Well done all those people who read the instructions!
