I followed the instructions for my first activation. For subsequent
activations I remained logged on, clicked on Access, submitted
the access token, and got an error message!
<<Access token was not found. Click here to activate an
institutional account.>>
I did as suggested and clicked through, repeated the Institutional
account activation process, ie: yes (to the agreement), submit
the activation code. I found that that was all that was necessary.
It wasn't necessary to re-enter IP range(s).
Each time a code is activated, a link is added to the "Profile". Now
I just have to find the new URLs...

Have others received emails about MIT Press journals moving from ingenta?
I've just received 6 of these (probably more to follow), detailing a long
complicated procedure to activate our subs., consisting of registering,
logging in and then activating. Each has a different customer number and
appears to require the creation of a different username and password, and
repetition of the procedure, for each one.
What a nightmare!
