Thank you for much for your helpful guidance, Steve.
It turns out that our problem with the second level feat had to do with two
things.
First, somehow, some of our first level feat had images which were not in
'nifti' format.
Second, somehow some of our first level feat did not have the 'reg_standard'
directory.
So, we had to manually run featregapply separately in order to finish up
what the first level feat had left out (without any warnings or error
messages).
Hope this helps others with the same problem.
