By coincidence, I was recently reading 'Archaeological displays and the public: museology and interpretation' ed. Paulette M. McManus, 1996. You could translate this to an archival context. They seem to recommend the format Title, Date and Material, Museum details, Commentary, Acquisition details - although others suggest Date first, with Date and Material in larger font so that they stand out from the 'hard' information, including museum number.
