In Excel's "Chart Wizard", at a certain stage you have the option
to include "data labels"; but there doesn't seem to be any way to
set these labels to be what you want! (The nearest thing you get is
"Show value" which labels each point with the value of its Y
coordinate).
Given that your graphing needs (and this may not be the only occasion)
are trying to escape from the confines of Excel's limited abilities,
perhaps you should consider creating your graphs in some other
software with a decent repertoire of capabilities, and then importing
the result as a graphic into (say) Word if that's what you need to do.
