Cathy
I would suggest posting this question on SRMSNET listserv (Survey Research
Methods Section of the american statistical association). I had a fabulous
response from the list to a question I put on response rates.
Good luck

Subject: Query - acceptable response rates to thirs party distributed
questionnaires
Dear Allstat list members,
Literature from the 1990's suggests the average response rate for mail
surveys ranges from 60-70%. Reviews of academic studies by Yehuda
(1999) and Asch (1997) support this figure.
Additionally, while a vast amount of literature is available on how to
increase response rates to postal questionnaires there is little evidence on
the expected response rate when questionnaires are distributed by a third
party e.g. pharmacists.
I am therefore in search of recent literature on the average response rate
for postal questionnaires distributed by third parties.
Would you be able to help?
