I am not Volkmar but venture to comment anyway: I do not think it is
necessary to do the combination manually. As far as I understand the
code (as to which John keeps pointing out that it is crude but works) it
does conditional dilatations and erosions, with a little smoothing
thrown in. So it starts in white matter and then moves outwards until a
certain threshold is not met anymore. It will happily accept two images
(I always select GM first, then WM but that may be irrelevant) and the
results look convincing.
