Actually, I am not exactly sure what you need for your analyses, but
for most ROI analyses, a combination of MRIcro for drawing ROIs and
MarsBaR for the stats works well for me, you may be surprised to hear.
One of the most common uses of MarsBaR is analyziing signal within
functional regions defined from individual subject SPM maps, and
combining functional ROIs with broader anatomical ROIs, so I don't
think you'll have a problem with assumptions of normal anatomy.
You might also consider this ROI toolbox, which has some very
sophisticated features.:

I would imagine that you will find something which will do most of
what you want, but would make the usual appeal to contribute useful
functionality you need to some publicly available toolkit so the rest
of us benefit,
