This is very similar to an example I had at the bottom of my earlier
reply to Marko, for finding either img or nii. Something like:
spm_select(inf, 'any', 'testing', [], pwd, '\.(hdr|img)$')
should do what you ask.
Note that the type must be 'any' rather than 'image' if you wish to be
able to select .hdr files. Note also, that in general this would be a
rather odd thing to want to do, as the reason spm_select(inf, 'image')
won't let you select .hdr is because functions like spm_vol don't work
on the .hdr files (they are happy with either the .img or a .nii).
