It is very common following trauma to the great toe in particular to suffer
from in growing toenails as the trauma site often gets infected and inflamed
hence the pattern of growth.
The other reasons you give are the most common for IGT.

anyone ever heard of an ingrowing toenail 5 weeks after dropping a box on
the foot?
i was under the impression it was something that developed with time such
as tight shoes, badly cut/short cut nails
