Dear Elizabeth
Surely the Climbié case is the classic?
www.victoria-climbie-inquiry.org.uk/ the summary report is nice and
short to read and its all downloadable, though it's a horrid affair and
will make your hair curl!
Hope that helps

Subject: Urgent - case examples needed by Monday!
Importance: High
I urgently need (by Monday...) examples of any published evidence that you
are aware of (in the media and/or in health care journals) that a BME
individual or BME group has received inequitable health treatment (by virtue
of their ethnicity) that has demonstrably resulted in death
or a deterioration in health.
I appreciate any help the network can offer.
Best wishes
Elizabeth
