From what you say, it seems very likely that smokers have long been dying of lung cancer, but as this was confused with TB (which I think was the main killer of the humand race) the true extent of the problem was simply not recognised till the 1960s.

Very easy to confuse the two, to this day. The scanning, endoscopic and pathological advances of the last 10-20 years make mistakes relatively unlikely, but it would by no means be impossible
