THE PROCESS OF PLANNING, DEVELOPMENT AND IMPLEMENTATION OF A GENERAL
PRACTITIONER WITH SPECIAL INTEREST SERVICE IN PRIMARY CARE ORGANISATIONS
IN ENGLAND AND WALES: job opportunities for social science researchers
with an interest in NHS organisational change.
We seek an experienced researcher and a part-time research assistant for a
study of reconfiguration of respiratory services in English Primary Care
organisations.
This project, funded by the NHS Service Delivery and Organisation will use
the redesign of respiratory care (specifically the role of General
Practitioners with a Special Interest) as an exemplar of workforce
reconfiguration in a chronic disease area not driven by government policy,
to provide insight into workforce dynamics within healthcare organisations.
The background:
Faced with the challenge of increasing health demands, particularly for
people with long-term diseases, the NHS is exploring new ways to provide
care. Many initiatives involve developing the skills of doctors, nurses
and other healthcare staff to enable them to adopt new roles, often
blurring traditional professional boundaries. For example, General
Practitioners with a Special Interest offer specialist advice normally
provided by hospital consultants. Little is known about the complex
process of reconfiguring services.
The case studies
Using screening interviews with a nationwide group of Primary Care
Organisations we will recruit six to eight at varying stages in the
process of reconfiguring respiratory care. We will observe the progress
they make (or do not make) towards their objectives by interviewing key
stakeholders (e.g. managers, doctors and nurses in primary and secondary
care, lay advisors). Initial interviews will explore existing services
and plans for development. Regular contact over a year will identify
events (e.g. meetings, training, crises, lack of funding) that will be the
subject of detailed investigation.
Six to eight patients in each Primary Care Organisation will keep
an ‘illness diary’ for a month which will record respiratory symptoms in
the context of their overall life situation, and the range of
formal/informal sources of support accessed Serial phone interviews and
focus groups will expand on the emerging themes.
The outcome
A national workshop of clinicians, managers, workforce planners and
patient interest groups will discuss the issues raised by the case studies
and formulate guidance on effective models for implementing workforce
change in Primary Care Organisations
The posts will start January and March 06 respectively. They will be
