As a freelancer I use Adobe Acrobat 7.0 professional. It comes complete with
accessibility features. Though not being visually impaired myself I know
friends who generally use these.
When saving document as pdf I can select to include accessibility features
(default settings) or not. The pdf file can then be converted to either
plain text or 'accessible text' (including comments etc.) Adobe also have an
integrated feature 'reading out loud' which I have tested. It sounds like
any other 'reading out loud' device.
I guess there can be problems depending on which version used when creating
the file or used when reading them. Also I don't know if the free adobe
reader comes with these features as I use my professional programme.

reading this before writing their support.
