We are about half way through a trial of IN fentanyl v IV morphine ( you cant get diamorphine in OZ )
we use 1 mcg/kg and use an atomiser , seems to be working well

Several of you have asked for our IN fentanyl policy.
I need to add that our experience remains very limited (only just introduced it) and it may need to be modified according to audit data.
The main issue compared with diamorphine is the large volume, which might be helped by some sort of atomiser, which we don't have, so we apply it to the mucosa very slowly, two applications per nostril (alternating).

We have been using nasal alfentanil in Barnsley for the past few months, with excellent results. A dose of 0.01 mg/kg has proved adequate for the vast majority of children. I can send you an abstract of our reported results ( which has been submitted ) if you want - just e-mail me off list.
