I've rewritten the manifest support, so that it now works without
relying on any file descriptors being inherited by child processes.
I've tested it in the four relevant cases on OS X -- we'll see what
happens on the other platforms tonight.
This has meant that the interface to the manifest has changed, for
those Makefile.am files which do fancy stuff at installation time: the
MANIFEST_FD variable has disappeared. This new interface is probably
somewhat neater than the MANIFEST_FD stuff was before. I've committed
consequent changes to

applications/generic/Makefile.am
applications/sgmlkit/Makefile.am
buildsupport/starconf/Makefile.am
libraries/ast/Makefile.am
libraries/pcs/dtask/Makefile.am
