Hang on a minute; I've seen fentanyl causing respiratory depression and/or
arrest, but have never seen these problems with ketamine. The words "tar"
and "brush" spring to mind...

It has happened to most of us I am sure, that something we have attempted
done to a patient has not always followed a predicted pathway and caused us
a few moments of stress (definitely happened to me).
The patient did survive, I did not see him personally as he was brought in
on the morning shift, however he did have a nasty upper limb injury.
