It means that a matrix was very difficult to invert properly.

Which SPM are you using? If it is SPM2, then the problem may be fixed by
increasing the amout of regularisation used for the bias correction. If it
is SPM5, then I'm not sure which part of the code is likely to produce this
error. It could be something to do with poor starting estimates for the
registration component though.
