I did download all the updates, but the problem still remains. I noticed
that the coordinates of the origin did change, but not the position of
crosshairs. It seems if you only do a translation, then the crosshairs
jump back to its original position. However, if you do a rotation, the
images are reoriented, but not as you expected.

This is a bug that I actually saw last week during the practical
demonstration
in our SPM course. I think it should be fixed if you download
ftp://ftp.fil.ion.ucl.ac.uk/spm/spm5b_updates/Updates_168.tar.gz and
unpack
it so that the existing files in your spm5b directory are overwritten.
SPM2 compatible images with .mat files were not being properly handled.
spm_get_space was ignoring contents of .mat files for single volume
files,
whereas nifti() was using the contents of the .mat files to over-ride
the
sform and qform matrices.
