Where additional information is required, I always start with a phone call
to the individual, that usually resolves most issues. If that is not
sufficient I then bring the individual in to see the OH doctor - I had one
case where I tried to get a GP report (because it was geographically
difficult to see the individual), but it was singularly unsuccessful, as the
GP declined to give me anything particularly relevant, so I brought the
individual in to see the OH doctor in the end.
All our students pay £15 (to Occupational health) when they apply for
fitness clearance. If additional costs arise (reports, OHP time), there is
no further cost to the individual, we absorb that into the contract.

Following paper screening of prospective teacher trainees, if it is felt
that further medical information is needed, our current procedure is to
request a GP report before inviting the student to attend for an appointment
with an Occupational Health Advisor. The students are expected to pay for
the GP report themselves (usually around £10 - 20).
I have a prospective student who is refusing to obtain and pay for the
report himself, and is questioning the fairness of this system (which I
believe was introduced as a result of the huge volume of applicants for
teacher training courses each year, many of whom fail to take up their
places on the course and so are therefore screened unnecessarily). I can see
the student's point, and am interested to know how others manage this
process.
