It might be a good idea to check with your Trust research governance lead to
be certain that the content of the questionnaire survey doesn't fall under
the remit of the NHS REC system. If the questions ask about staff practice
it might well do.

Being a new healthcare assistant librarian, I have been charged with the
dubious honour of conducting a library user survey. The questions are set
but all it remains for me to do is to ensure a good return! Has anyone got
any hints or tips that may assist me to create a good first impression to
