I remember about 3/4 years ago mention on the list of a systematic review in surveys - it might be in the archives. Some interesting tips I remember for getting for getting a good return were
Use white Stamped addressed envelopes
First class stamp for sending and reply
Coloured typeface
Have an incentive for return - prize draw etc.
We used all of the above and got a reply rate of over 30% - not brilliant but not bad
