Bugs should be reported in savannah:

Ideally you should register with savannah, but I think it also allows bug
reports from anonymous people. If you think something is a bug it's better
to report it, if it's a mistake it can always be closed. You can also ask
for changes and new features there, using the item group "missing
functionality".

I would agree that code should be commented and errors should be logged.
Sometimes it's hard to avoid the dependence on the format of command output,
but it should at least be documented.
