One of the clear loci of blame was that the PCT carried on none of the
memory of the PCG in relation to this...
thus they swung like a compass needle back to what they had been doing
before...
and despite still being unable to analyse figures to determine whether
they were a real difference, and whether the difference was due to
doctor factors or other factors they repeated their actions that had
caused much harm before.
That doesn't seem unlike PCTs in various respects, and reminds me of a
paper a while ago called "An organisation with a memory".
