I find this discussion very interesting. I have been in General practice
over 20 years now, and we have had a true vasectomy failure (never had
sterile semen sample collected afterwards), a false vasectomy failure (wife
had a one night stand with someone, semen was sterile), several coil
failures, and NEVER had a female sterilisation failure, nor has any GP I
have ever spoken to had patients with the latter.
So, how many of you have seen lap sterilisation failures, and how do they
compare in your practice to failure rates for these other methods?

Mirena has a lower failure rate than the first year of lap steri, after that
things even up I believe.
However menses seem to get heavier after lap steri and the irreversible
nature can cause depression.
I recently audited my coil insertion from 2002-2004 inclusive, and having
done 59 we had one failure, the mirena was never found and must have fallen
out along with 3 other patients who had their coils fall out(nearly all
mirena). More worrying though, the two pregnancies (one planned as well as
the one failure) both ended in miscarriage. Sample is too small to draw any
conclusions from obviously, however would be interested to hear anyone
else's experience, possibly off list.
Cochrane search was interesting as they found a very low infection rate for
any coil insertion and that copper baring coils had no higher failure rate
than mirena. The side effects of low to zero menstrual flow is the usual
reason that I fit them though.
