Yup - that's the one I filled in. Fairly comprehensive DP statement, though
I obviously didn't read & absorb thoroughly at the time, or I'd have
remebered it yesterday!
Although each person will get their own E111 / EHIC after 'dad'(or 'mum')
completes the form on behalf of the family - its not clear whether the
spouse / partner can opt out of being part of the 'family' application.
Even if 'dad' or 'mum' deals with it for dependent children, shouldn't the
adults make their own choice?
Probably the vast majority won't notice or care about the DPA issues. Those
parents that do may well be surprised at the thought of seeking consent
from their dependent children!
