An error in a chapter has been drawn to my attention which I will pass
on. It was not present in the author's proof and has appeared somewhere
between the final proof and the actual printed work.
The book is "Practical Radiation Protection in Health Care" edited by
C.J. Martin and D.G. Sutton. I have a chapter 19 on Lasers. The formula
for nominal ocular hazard distance (NOHD) shown in equation 19.2 is
incorrect. The square root sign is missing. There should be a square
root sign over [4 P / pi MPE]. In Box 19.3 in the sample calculations,
the same error appears on the 4 occasions that the formula is cited. The
results quoted are actually correct because the square root was used in
the calculation.
The publisher, Oxford University Press, has been notified of the
error.
