The authors of the HAD scale have handed it over to a publishing
company, Nfer-Nelson, for distribution and they supply copies of the
questionnaire plus analysis charts and instruction manual. For a cost,
of course!
My understanding, from similar work, is that it would be breach of
copyright to make your own copy.
It would be worth checking with the original author. If the nurse is
doing a small scale piece of research for a course he may be happy to
allow this. The following paper has Snaith's email, plus a section on
"Obtaining the HADS":

Hi All,
I have a copyright question- Marie Ford, who I work with, is off at the
moment, but she would be perfect to ask.
A Nurse has a copy of the-
Hospital anxiety and depression scale
Which is a basic questionnaire to scale depression. She got her copy
from a text book. She wants to give this questionnaire to patients but
is worried about copyright.
I have found online copies of the Hospital anxiety and depression scale
on many different websites, including www.patient.co.uk.
I would be tempted to say that she could type it out herself and give
out as long as she referenced it, but I could be totally wrong about
that.
Any advice would be greatly received.
