Yes, the transcriptionn is not accurate enough, so one would be reluctant to be dogmatic about the first line, but then:
Nomina constabulariorum (The names of the constables)
Juratus Thomas Chount Constabularius Libertatis de Halestocke (presumably) (Thomas Chount sworn Constable of the liberty of Halestock (or whatever this place is).
That's a quick and dirty rendtion.
