If I was offered Human Tetanus Immunoglobulin I would refuse. The
history of human pooled infusions is too littered with stray viruses for
me to feel comfortable with this. How many seriously contaminated
wounds would have levels of Cl Tetani significant enough to cause
tetanus after proper wound toilet and metronidazole ? I would have to
think VERY carefully before persuading someone else to have HTIG.

Wasn't there a tetanus case in the literature recently, in a fully
immunised Israeli soldier?
I'm inclined to think we should be more generous with our expensive
immunoglobulin. I know of some departments that probably contravene the
guidelines for economic reasons.
The bottom line is, in a tiny number of cases, vaccine alone is NOT
enough.
