I think a copy of the Perkins chart is in <John Foxe and His World>. It's

I was wrong--once you have "the decree of the reprobate" and God's hatred
against you, you may receive a calling, but it is "not effectuall," and
although you may "yeeld" to God's calling, enjoy general illimunation,
penitence, temporary faith, "a taste," and "zeal," you are bound for
"relapse," damnation and "death eternall in hell." The elect on the other
side of the chart can have all kinds of doubts, concupiscence of the
flesh, and even be enemies of the life eternall if they're on path B. It
looks like you can make it into heaven with only the decree of election,
an effectual calling, and a life of doubt and concupiscence. -Dan Knauss
