There was a very helpful article of this nature last year that has a great biblio too - it is not youth specific but would still be relevant -

Also - the AAMR has some ethics statements about it on their website and many local/state/provincial advocacy, policy & service organizations have developed excellent practical tools for carrying such research out in respectful, accessible ways.
The University of Toronto has a group called Quality of Life Research unit - they have also developed a method around this (Ivan Brown, R. Renwick ...).
