The price very much depends on the amount of analysis required, which varies from project to project. If you've got something in mind, it might be best to contact suitable organisations directly for quotes - e.g. members of the Market Research Society include many quantitative & qualitative research companies, or the psci-com archive includes several links to previous web questionnaires.
If you're going to do the analysis yourself, and are just looking for the development of the web pages and database back-end, you're probably best contacting web design-type companies, using online services such as that mentioned by Avril, or there may even be some open-source scripts you could use (anybody know?). With the role models study, the costs of creating http://www.opm.co.uk/royalsociety.htm are included with a larger staff costs, so I can't give a figure.
