We have a form for this which comes as part of a leaflet. As an organistion
we often work with volunteer groups tidying up bits of the countryside etc.
The basic principle is to ask volunteers to declare an absence of
significant and uncontrolled medical problems as the work is often of a
manual nature and involves being around water courses. The questionnaire is
provided by our employee responsible for supervising the volunteers. The
form does not require medical details as we don't wish to get into the
confidentiality debate but does allow the supervisor to turn someone away
until they can present some information stating they are ok, if they have
declared something worrying. Ofcourse the volunteer can be less than
truthful but that is always the case and our legal department are happy with
the process
