Carol,
Could you enlighten regarding the 'the type of forklift truck'
We have several types, I'm not sure what the difference is, where can I find
this information?

Yes, depending on type of lift truck and it's important to know which type,
depth perception, PMH of cardio-vascular conditions including hypertension.

I do quite a bit of these for 2 OH providers.
Generally:
Medical history-exclude neurological problems, ,musculo-skeletal/balance
problems

Vision: distance is importance, and some would say binocularity/steropsis,
along with peripheral range of vision. If you have a box screener this
pretty well does it . Otherwise it Snellen for distance and visual fields by
confrontation, a skill many OHAs have not commonly acquired.

Movement- arms legs neck back , balance.
BP, pulse

Urine test for Diabetes

Hearing, audiometry or voice test-enough so they can hear warnings.
Height and weight usually done- but as long as they are not so large as to
be immobile there is no real "cut off"
Refer on to GP or OHP if you have one for queries
Some firms do /peakflow/colour vision/near vision

Does anyone do anything else?
