good medical practice for occupational physicians.
Other references are the GMC publications on ethical issues including good

Hand written notes should be dated, written in black, easily readable, and
signed.
We have a policy on the quality of occupational health records and I
regularly audit the quality of notes in the Trust where I work. They will
not pass the audit if they are not signed and dated.
If records are not dated and signed then they will not stand up to criticism
and will be defendable in a court of law or industrial tribunal as it will
not be clear when they were written - prior to or after the complaint /
litigation.
The BMA guide and Faculty guide indicate that a doctor should not use the
term Occupational Physician unless they are an accredited specialist with
the MFOM or possibly qualified to AFOM level.
