Simple disclosure - you need to advise work on modifications "a medical
condition has come to light which ...." and you can do it without
disclosing the medical condition.
Is it work-related? eg DSE-related or lifting & handling related? If
so, and this job involves similar work, I think you should explain to
the employee that further strain might well exacerbate the condition and
you need to act to protect them and that it is helpful to disclose
limited medical information to managers to help them make adjustments.
Let her stew on that and see her for follow-up - they usually agree to
diisclosure.
I don't see that you need to discuss what was in the pre-employment
form with managers - it's not your role to be a policeman.
