I am an Occupational Health Nursing Advisor and have been told by my
employer that I must get managers to sign a consent form before they come
to see me (they are consenting to say they are happy to come and see me
and are happy for me to povide a written report back to management if
required)
I dont have a problem with this in theory but I am concerned about the
validity of the consent form and whether it is suitable for purpose. I
have been told that I must use the consent form that is used throughout
the whole business. This has information such as National Insurance
number (which I thought was not supposed to be used any more), GP and
specialist details. Within my current contract I am not in a position to
request a report from a GP and therefore I do not feel this is a suitable
consent form.
Most importantly the consent form states that the individual is consenting
to a face to face consultation with a doctor. Since I am an Occuaptional
Health Nurse I do not feel that this consent is valid. I would be
greatful to hear other peoples views.
