Thanks to everyone who replied. I'm much clearer about it now!
Comments included:
"In practice would your report and portfolio end up in one binder? [Yes, I
am] If so 2 copies seems unnecessary and cross referencing could be
advisable".
"I chartered in June under 2002 regulations. I only put one copy of these
documents in, as numbered entries in the portfolio section".
"I think it means (and apologies if I'm wrong) that if you in your report
mention the CV 6 times, keep referring back to the same CV (which you will
have numbered, for example 6), rather than sticking 6 CVs in the portfolio!
I'm putting my CV and aims etc. in the portfolio, I don't think the report
should have anything attached to it!"
"I think that as these docs are in the Portfolio there's no need to have
them as appendices as well. I was just going to refer to the Portfolio docs
where
appropriate in the Report".
"Think of the statement as a document without appendices and the portfolio
as the
appendices (with "attitude" - ie: they aren't just appendices they are a
portfolio, but they are directly relevant to the contents of the written
statement)...."
