The PCTs in my area have been addressing just that problems for the past
1-2 years. We started with intra-practice SEAs that were to be discussed
at their in-house training sessions. These soon picked up
primary/secondary care and GP/Pharmacy problems. We encouraged practices
to try & explore these areas but it can be difficult to have a one-sided
conversation. One of my practices was told that it should buy certain
resuscitation equipment by the local Maternity department after a
patient fainted - there had been no contact with the practice.
I used that as an opportunity to have an interface SEA meeting the
representation for all those concerned at that practice - with a much
more sensible outcome and greater understanding from all of everyone's
concerns.
We now have a regular Interface SEA group between PCTs and the local
Acute trust and are starting to monitor problems with a view to
assessing 'Risk' and developing solutions.
With regard to GP/Pharmacy SEA my own practice has invited the local
pharmacist to one of our practice meetings. The PCT has also appointed a
Clinical Governance Pharmacy Facilitator whose role is to help develop
clinical governance at pharmacy level and also facilitates resolution of
such SEAs. Of course the Dentists appear to be looking after themselves,
so far so well, but we have just appointed a facilitator for local
optometrists
It appears so far that practices discus operational problems at their
SEA sessions frequently making simple but effective suggestions for
system change. Secondary care generate many more 'events' but many of
them appear to be irritations and interpersonal sniping - this saps the
clinicians energy and enthusiasm reducing their learning value. The
local trust does log and catalogue all these events trying to look for
trends but tend to focus more on complaints and litigation in terms of
system change.
