Thats exactly my point - some people need higher doses. The evidence from a
massive study done on US Veterans showed that the addicts who did best were
those in the higher dose ranges for Methadone. Presumably your worry comes
from the unrealistic guidelines published for shared care. I don't have
anything to do with this but most of my patients on maintenance are on
50-70mg daily. You cannot run a holistic patient oriented drugs service if
you are more concerned with selling on (which may well happen in the private
sector to fund the payment to the doctors fees) than providing the needful
regimen for each person. Daily pickup from pharmacies is good for titration
but no good long term as it removes any responsibility from the patient for
management of their drug taking which is waht it is all about - the
therapist and pharmacist cannot force an addict to comply they can only do
that for themselves. I believe that supervising dosage has no value except
as I said for safe titration.
