One tool, which I don't think has been mentioned in
this thread, is the Spag that comes with the (paid for) plusFORT
from Polyhedron. This does some f77 to f90 conversion including
fixed to free format, f90 declarations, intent for arguments,
do while, exit and cycle, etc.

It all so has extensive options to control the case of: local
variables, common variables, dummy arguments, parameters
and other symbols. For example, you can have uppercase
for common variables and lower case for local ones, if you
wish. This should satisfy most tastes in this area.
