Producing population-based morbidity information from British General
Practice has been a holy grail of health care for many years. Learn about
the Chronic Disease Register in Ealing Primary Care Trust and how it is used
to improve the management of chronic diseases at the individual and
population levels. The register has been running since 1998 and has amassed
much practical experience both of benefits and difficulties.

The Chronic Disease Register (CDR) is the largest and most complete
population-based disease register in the world. The overall objective of the
CDR is to improve the care delivered to people with Coronary Heart Disease
(CHD) and Diabetes Mellitus within West London, through the provision of
information technology and systematic administrative and clinical processes
in an integrated manner across primary, secondary and tertiary care. This
includes the extension of the existing service with a fully implemented
web-based CDR, in support of patients with CHD and Diabetes. It also
includes the provision of support to General Practice under the new GMS
contract, and diabetic retinal screening support.
