Over in the UK, Andy Horwood as been advocating the use of forefoot
extensions - sulcus length - as a dynamic orthoses treatment modality. It
is referred to as propulsive phase posting.
Valgus posted sulcus extensions seem very effective at 'facilitating'
lateral column stability and consequently transverse and medial arch
function i.e hicks windlass and fore foot stability.
In my own practice, I have used it effectively to treat laterally unstable
patients (Max evertus test), especially where knee pain is present. It is
also very effective in the 'toe' runner where the use of rear foot posting
is ineffective.
I am intersetd in whether this approach is used in the US or is Andy
Horwood as much of an innovator as I would like to beleive.
