I too have noticed this problem and now attibute it to the change in foot
loading assocaited with fatigue. This has been documented in the research
but no way of changing it has been suggested.
I now give my long distance runners eccentric muscle exercises to decrease
fatigue and this has been approx 50% successful- primarily for tib post
and the long flexors and peroneals.
I also give a soft poron 'throw away' top cover to be placed on top of the
orthoses prior to a long run. Again with some limited success.
