Adam,
One of the possible causes is Anterior compartment syndrome. Usually
related to an equinus in which the anterior tibial muscle has to work
harder to dorsiflex the foot at heel contact. The muscle swells and this
causes pressure on the anterior tibial nerve.
It is rare in a runner. Haven't seen it in a runner.
The diagnosis is made by checking the compartment pressure.
