You could contact Transco (now National Grid Transco) as they used the Chester step test to assess fitness for BA, from my own experience step height choice should be more to do with the individuals CV fitness than their height, as the lower step may not provide enough of a challenge to an individual who undertakes regular CV exercise.

Query:
What fitness test regime ( e.g. Harvard step test) do any of you guys use to
determine fitness for employees using breathing apparatus, both high and low
aerobic users?
The employer is an utilities company (water) and the BA users are sewage
operatives.
If you use the Harvard test, what is your step height ( recommendations vary
between 45cm and 60cm) and do you lower it for people of small stature?
